Policies and disclosures
To contact the office regarding setting up an appointment, please send an email with the following information:
-Your insurance
-If you are currently taking psychiatric medication
-Your diagnoses from past providers, if known
-If you are transferring care from another psychiatric provider.
You will be provided a link to a patient portal once we have discussed the above and decided to schedule an intake. Intake documents are due 48 hours prior to the appointment or the appointment will be cancelled.
Some information that may help you decide if we are a good fit:
Please be aware that we will not start or continue prescribing Benzodiazepines (Alprazolam, Diazepam, Lorazepam, Clonazepam). If you are taking these medications regularly, a part of your treatment will be tapering (decreasing) and stopping these.
We treat adults only (18 and over) in the practice.
There is a required $100 deposit to schedule an intake, and this is to discourage people from making an intake appointment and not showing up. It will be used for future deductibles and/or copayments, or refunded if you don’t owe copayments or deductible. It is not returned or refunded if you do not come to the appointment (you may reschedule up to 48 hours prior to the appointment).

Payment and Insurance
We are able to take insurance as well as private pay.
If you have Medicare or Medicaid we cannot accept cash payment and you will need to see a provider who is in-network with Medicare or Medicaid.
